Against this background it is important to lay out a set of principles, aims and objectives which will guide staff and set the vision for a community based service.

Although these principles are expressed in terms of what the service should achieve for the Service Users, the fundamental requirement of the service is that it should be 'person centred'.

The services provided should be shaped around the unique needs of the individual Service User at the point of delivery.

It is essential that the lead responsibility for each phase of care and support is agreed between the Service User or their advocates and Lomack Health - Domiciliary Service.
